CODE 4 HOME CARE
A key programme of the Foundation which provides health services to homebound persons at their residences. The services are delivered by a team of doctors and nurses.
The care is targeted at frail and disabled older adults with chronic diseases and mobility impairment, who are homebound.
What does Code 4 do for its patients?
Homebound patients often have complex health needs with multiple medical problems that are closely intertwined with psychosocial problems. After a comprehensive assessment is made at the patient’s home, a management plan will be developed to meet the patient’s total care needs through a range of services. This may include:
• Management of chronic medical problems.
• Management of uncomplicated acute or subacute medical problems.
• Educating the patient and caregiver on the patient’s illnesses and the managementplan.
• Providing caregiver education with regard to various aspects of nursing care e.g. prevention of pressure sores, proper feeding techniques, etc
• Performing nursing procedures e.g. wound care, care of enteric tubes, tracheostomies, enterostomies, urinary catheters etc.
• Palliative care for patients at the end of life
• Certifying disabilities for ELDERSHIELD / IDAPE payouts
